The Life House

For everyone's safety, anyone who plan to attend a function at The Life House Community Church, Inc. must sign and submit the "Attendee Consent Form" below. 

Building admission is limited to those who:

1) have no Covid-19 symptoms

2) do not have a positive coronavirus test result  

3) have not been exposed to anyone with coronavirus within the last 14 days

4) have a temperature reading less than 100.4

5) have signed & submitted the below Attendee Consent Form

6) have submitted the online In-Person Congregation RSVP

ATTENDEE CONSENT FORM

As we all know, the Coronavirus/COVID-19 has become a global pandemic.  Our communities and our church are not exempt from this threat, and The Life House Leadership has subsequently implemented a more rigorous safety and disinfectant protocol.  Some of the key focus areas within this protocol are: the thorough disinfecting and/or germ protection of audio-visual equipment, the thorough disinfecting of all high-touch surfaces, installation of plexiglass desk screens, the closure of non-essential rooms and offices throughout the building, and the mandatory use of face masks and gloves (based on volunteer service type).

We strongly believe that health and safety is a shared responsibility.  Therefore, any volunteers or program participants who have a fever, cough, respiratory symptoms or have recently been in contact with someone who has been sick, will not be permitted to serve.  And members or guests who have these symptoms, or who have been exposed to persons with these symptoms in the last 14 days, are not allowed to be in attendance at any Life House in-person services or events.  Furthermore, take note that The Center for Disease Control (C.D.C.) advises that senior citizens and those with underlying health conditions are especially vulnerable to this disease.  

 

As a note, touchless thermometers will be used to take body temperatures of all attendees.  Those with a temperature of 100.4 or greater will not be allowed to enter the building and/or serve. 

While The Life House leadership and supporting ministries have taken enhanced safety measures and adopted C.D.C. recommended guidelines, it is important to know that an inherent risk of exposure to COVID-19 exists in any public space.  By visiting The Life House Community Church, you knowingly and voluntarily assume all risk and responsibility related to exposure to COVID-19. 

 

Typing in your first and last name below serves as your digital signature on this document, and confirms that you do not have nor have not knowingly been exposed to any of the above mentioned sickness symptoms.  Your signature is required before you can be allowed inside of The Life House facility, or be able to serve indoors or outdoors as a Life House volunteer or program participant.
